For those who criticize images like this for being too "sick" or "ugly," the point of such images is to provoke thoughts in those who see them. Art is not simply about making aesthetically sound visual representations of things, art is about provoking thought and reaction.

I suspect (though I might be completely wrong) that this image depicts the victim of an explosion of sorts. With no context at all this image is slightly lacking in meaning but some people will certainly get strong feelings from it (imagine someone with a relative who was caught in a terrorist bombing, for example.) As such, it might be a great image for anti-terrorist propaganda, or perhaps a work safety promotion of sorts.
